Telehealth Informed Consent

By submitting your intake assessment on the RENVA platform, you acknowledge that you have read, understood, and consent to the terms of this Telehealth Informed Consent.

1. What Is Telehealth

Telehealth refers to the delivery of healthcare services using electronic communications and information technology. RENVA facilitates telehealth encounters between you and licensed independent clinicians who evaluate your intake information, answer clinical questions, and — where appropriate — prescribe medications.

The telehealth services provided through RENVA are subject to the same professional standards and legal requirements as in-person healthcare services. Your clinician is a licensed independent provider in the state(s) where they practice and where you are located.

2. Asynchronous Care Model

RENVA operates an asynchronous telehealth model. This means that your clinical encounter does not occur in real time. You complete an intake questionnaire and submit it for clinician review; your assigned clinician reviews your information, makes a clinical determination, and responds via secure message or prescription issuance — typically within 24 hours.

There is no live video or phone appointment. You communicate with your care team through the secure RENVA platform. Response times may vary; please plan accordingly and do not use this platform for urgent or time-sensitive medical needs.

3. Benefits and Limitations vs. In-Person Care

Potential Benefits

  • Convenient access to licensed clinicians without leaving your home.
  • Reduced time and cost compared to traditional clinic visits.
  • Discreet, private care for conditions such as hair loss.
  • Continuous monitoring and AI-assisted progress tracking over time.

Limitations

  • Your clinician cannot perform a physical examination. Certain conditions require an in-person evaluation and cannot be appropriately treated via telehealth.
  • Diagnostic capabilities are limited to the information you provide. Inaccurate or incomplete information may result in an inappropriate clinical determination.
  • Telehealth is not appropriate for emergency situations. If you are experiencing a medical emergency, call 911 immediately.
  • Not all health conditions can be treated through RENVA. Your clinician may determine that in-person care is more appropriate for your situation.

4. Technology Risks

Telehealth services are subject to technology risks that may impact care delivery, including but not limited to:

  • Interruption, loss, or failure of internet or platform connectivity.
  • Security breaches or unauthorized access to transmitted data despite our security safeguards.
  • Delays in message delivery or prescription transmission.
  • Technical errors in the platform or third-party services on which RENVA relies.

We implement industry-standard security measures including encryption to protect your health information. However, no digital system is completely immune to risk.

5. Your Responsibilities

  • Provide complete, accurate, and truthful health history information during intake and in any subsequent communications with your clinician.
  • Disclose all current medications, supplements, and known allergies.
  • Review the information provided about any prescribed medication, including side effects, before beginning treatment.
  • Follow your clinician’s instructions and notify them promptly of any adverse reactions, side effects, or changes in your health status.
  • Seek in-person medical care for any emergency, acute condition, or concern that your clinician determines is outside the scope of telehealth treatment.
  • Keep your RENVA account credentials secure and not share access with others.

6. Emergency Situations

RENVA is not an emergency service. If you believe you are experiencing a medical emergency — including but not limited to chest pain, difficulty breathing, severe allergic reaction, stroke symptoms, or thoughts of self-harm — call 911 or go to your nearest emergency department immediately. Do not use the RENVA platform to request emergency assistance.

7. Right to Withdraw Consent

You have the right to withdraw this consent at any time. Withdrawing consent will discontinue your telehealth services through RENVA. To withdraw consent, contact us at darrin@renva.health or cancel your subscription through the dashboard. Withdrawal of consent does not entitle you to a refund for services already rendered.

8. Privacy

Your health information collected through the RENVA platform is protected in accordance with our Privacy Policy and, where applicable, HIPAA. Please review our Notice of Privacy Practices for a full description of how your health information is used and disclosed.

9. Acknowledgment

By proceeding with your RENVA intake assessment, you confirm that you:

  • Are 18 years of age or older.
  • Have read and understood this Telehealth Informed Consent.
  • Voluntarily consent to receive telehealth services through the RENVA platform.
  • Understand the benefits, limitations, and risks of telehealth care as described above.
